You can generate a ftl file for the human task created.This is eclipse editor (2nd image).
Same can be done in guvnor repository also. Check the 1st image and click the button under red circle. Hope this answers.

I have generated the files using the eclipse editor and when I view it in eclipse, the fields appeared. But when I add the process and ftl files to guvnor and rebuild my package, the forms don't appear when I try running the process in jbpm-console. Any idea why? Thanks.

1. Generate the forms in Guvnor.
2. From the Assests in guvnor, open the taskform ftl, it will be in the name <taskformname>-taskform. Click open, upload the file what you have generated in eclipse. and build the package, this should resolve.
